(CYG_EXC_CONTINUE_EXECUTION): Define based on standard Windows definition.
(CYG_EXC_CONTINUE_SEARCH): Ditto.
(exception::handle): Move andreas detection earlier.  Make comment clearer.
(signal_exit): Set core-dumped flag.
